Navigators use the platform to automate digital check-ins, gathering information about the resources received, and resources they need now. With each check in response, AI interprets needs and finds relevant resources for each patient.Personalized Resource Connection:
Within the We Are Here platform, AI is used to match the list of patient’s needs with thousands of potential resources. Navigators review the recommended patient resource list to send to the patient now and schedule some for a future time. Navigators also search the We Are Here database for additional resources. Every day, this resource database is updated with the latest resources and information to ensure that patients have the best set of resource options that are available.Actionable Insights for Action:
At the patient level, navigators view the dashboard to see survey responses to understand how with patients being supported. Beyond sending resource lists, navigators have visibility into the holistic needs of patients to determine which needs have been addressed (by them or other care navigators in the health system) and which needs are flagged as outstanding triggering proactive outreach with additional resource options.Guided Needs Assessment:
